Step 3: For individual or hospital insurance, fill the proposal form in detail. Do not claim you have only qualified staff or that you have only qualified doctors. Write down names of all places where you work or could be called to work in. Write names of all doctors who work or could ever be called in emergency to work in your hospital

Step 4: Calculate your premium including MLAG membership charges if you are not already member. Hospital, husband, wife all will need to be members of MLAG separately.

Step 5: Send the hard copy of both the forms and cheques to ALL CHEQUES/ D/D SEND TO THIS ADDRESS
